Congratulations are in order for the staff of the GRACE Community Clinic! We have been awarded GOLD Plus Recognition for the seventh consecutive year for the Target BP Program through the American Heart Association! In summary, this means that 87% of our clients with a hypertension diagnosis have been able to keep their blood pressure in check per the American Heart Association standards for blood pressure control.

The Clinic provides patients with individual physician management for their chronic diseases, medication management, and patient education. GRACE teaches them how to self-monitor their blood pressure daily and provides the tools to do it, i.e., Validated Blood Pressure Machines, Blood Pressure Logs for their self-management, and 1-1 patient education. These patient blood pressure logs are brought to each physician and nurse visit to help providers in their patient assessment and medical management.  We educate our clients regarding the importance of blood pressure control to prevent other conditions such as heart attack and stroke.

The GRACE Community Clinic wouldn’t be able to do all we do without our incredible community. In the last fiscal year, the Clinic provided 5601 patient visits during 739 individual clinics. These clinics are only made possible by our 104 active volunteers, including numerous medical providers, who volunteer each month to help us meet the healthcare needs of our community – last year, they gave GRACE 2,489 volunteer hours.

As a final note – the Clinic has begun to hold vaccine clinics through partnerships with our local Walgreens, Americares, and the Tarrant County Health Department. We appreciate our partners so much and the huge effort that they are providing for our preventative care initiatives for our clients/community.
