FAMILIES ARE MAKING NOTICEABLE CHANGES

Thanks to our partnership with the Grapevine REC and the H.E.R.O (Helping Everyone Enjoy Recreational Opportunities) program, clients who wouldn’t otherwise be able to participate due to financial hardship are making tangible progress.

Patients are challenged to reach a benchmark of a 10% health progress rate each quarter. Nearly three-fourths of our diabetes patients were able to decrease their hemoglobin A1C levels and reach their goal. More than half also increased their blood pressure to the target range, and nearly half decreased their BMI by at least 10%.

LIVES ARE BEING CHANGED

The Community Clinic was able to locate prosthetic services for a new client who recently had a below the knee amputation due to old injury and severe diabetes. This client has taken advantage of the Clinic’s Diabetes Classes, and has graduated from the Clinic’s Cooking Matters nutrition course.

In only three months, this patient has decreased his HgAIC from an 11 to 6.8, which is incredible. Thanks for your support, he has enjoyed renewed energy and sustained health.

CLINIC WELCOMES NEW VOLUNTEER PHYSICIANS

GRACE welcomed two physicians to its team of volunteers: Prasuna Dubagunta, M.D., Internal Medicine, and Tammy Polit, D.O., Family Practice.
