How to Get Help

Connect with a Care Manager to assess your medical concerns at the GRACE Community Clinic. Individuals seeking health care must reside in the GRACE service area and meet criteria to be eligible for services.

For the fastest response and to determine your eligibility, schedule an appointment below and upload the necessary documents prior to your visit. Walk-ins are available between 10:00 a.m. and 6:00 p.m. Monday thru Friday and one Saturday each month, but scheduling appointments is strongly encouraged.

A Holistic Care Network

Our Clinic staff work tirelessly to meet all your needs internally. However, as appropriate, patients are referred to Tarrant County’s John Peter Smith Health Network and other specialty providers for services that cannot be performed at our clinic.
